How can this book be described as a "series of missed opportunities"?

I don't see that there were "missed opportunities" as much as there was a need for both of them to grow up before they could settle with each other. They were always friends...the best basis for a good relationship but Dexter seemed unable to recognize that he really cared for Emma.

It struck me as a I read about Dex's letter from India that never arrived and the time Dex tried to call Emma but she was already out on her date with Ian, what might happen if these "missed opportunities" were not missed but resulted in actual conversation between the two of them. They are so immature and unable to face their emotions or true selves for so long that I wondered what would have happened if luck forced them to face each other. Perhaps they would have continued to be two ships passing in the night, but I got the sense that things might have gone differently if they had been in the right place right time earlier in their lives.

They were psychologically unmatched in maturity levels. Emma grew up faster than Dexter and wanted a more stable life. Dexter was more interested in himself and his immediate needs for fame and hedonism at the time that Emma would have wanted a relationship. They 'missed' in timing until they married.
