At times Leo tends to put Emma on a pedestal. But when the truth finally comes out, she says to him, "Life made sense again, when I met you, Leo. I remembered why people wanted to live." Do you think he has ever believed she loves and needs him every bit as much he loves and needs her?

RE: Do you think Leo has ever believed ...

This is easy, yes, but as many have said Emma led a very complicated emotional life. I have seen many women with post partum depression suffer-yet they seemingly had no issues before and it took a lot of medications and therapy to get them to a good place.

I agree with Barrye. Emma's lie or omission regarding her pregnancy and adoption somewhat mimics the misrepresentation - or lie - of Leo's parents. When Leo learns the facts of Emma's situation and can understand and relate to her feelings and actions, he realizes just how very much she has suffered and needs him. And indeed, he came to help find Janice and was very supportive and accepting of Charlie.

No, I don't think Leo has ever believed Emma truly loved him. I think he's insecure as a result of his past and has trouble accepting that people love him enough to not leave him. I don't believe this changed by the end of the novel.
